Annual Reappointment for Medical and Professional Staff (June 2020-June 2021)

Payments for your annual reappointment are now available online. Submission of both an application and successful payment of your fees using the form below are required to maintain privileges at CMH. The fee for ‘active and associate’ category staff is $150 and for ‘affiliate and courtesy staff’ it is $50. If you are part of the ENT or Urology Regional Call program as courtesy staff at CMH you are exempt from the $50 fee and do not need to complete this payment form.

Refund Policy: Annual reappointment fees are non-refundable.
Reappointment fees are not taxable, no HST will be added. A late penalty of $500 will be applied to any reappointment application submitted after the deadline of February 15, 2020.
